Public health in a historical perspective

When initiating analysis of the public-health situation in the former Soviet republics back in the 1990s, one of the special interests of SCOHOST researchers was to find out to what extent the current situation could have been caused by the circumstances of the Soviet era.

Out of these humble beginnings a greater interest for the historical dimension of public health questions developed, and many kinds of scientific works have now been produced using this perspective. Types of studies include:

  • Assessments of the impact of historic circumstances on individuals' current state of health, especially in the Leningrad Siege studies, which show that long-term starvation has far-reaching implications for health decades later (see Sparén et al. 2004, Vågerö et al. 2013)
  • Studies on the historic prevalence of specific causes of death (see Stickley et al. 2007, Stickley et al. 2009, Stickley et al. 2011, Jukkala 2013)
  • Studies on relationships between phenomena over time (see Stickley & Carlson 2005, Stickley & Pridemore 2007, Norström & Stickley 2013)
  • Studies on historical changes in the geographical dispersion of homicide (Stickley & Mäkinen 2005), suicide (Mäkinen 2006), and alcohol consumption (Mäkinen & Reitan 2006)

Overall, the studies show that history plays a vital role in public health, both as a precedent that influences the current situation through path dependency and narrowing the range of current alternatives, and as a "natural experiment" that allows us to compare a current situation with another one in a different time, even following the path that it has taken to reach the current situation, an advantage that time-bound studies have over cross-sectional ones.

Social relations and health

In recent years there has been a growing interest in social relations and social capital in the field of health research. Social capital is generally thought to have a positive impact on health.

For an overview of the concept of social capital and its different forms in relation to health, see Ferlander (2007).

However, there have been relatively few studies on social capital and health in Eastern Europe. To address this gap, within this research theme the relationship between social capital and health is studied in Russia and Ukraine.

  • More specifically, the aim is to investigate:
  • The relationship between different forms of social capital and health.
  • Gender differences in the relationship between social capital and health.
  • Mechanisms between social capital, gender and health.

Both physical and mental health is examined with data from three surveys: the Moscow Health Survey 2004, Stakhanov Health Survey 2009 and Yaroslavl Health Survey 2010-2011.

SCOHOST has already conducted several studies on social relations and social capital in Moscow. Among other things, the results have highlighted how institutional trust is very low among Moscovites (Ferlander & Carlson 2005; Stickley et al 2009). Studies have also shown that different forms of social capital influence binge drinking (Jukkala et al 2008), psychological health (Kislitsyna & Ferlander 2008), self-rated health (Ferlander & Mäkinen 2009) and victimisation (Stickley & Pridemore 2010) in Moscow. All studies show significant gender differences in the relationships.

Urban environment and health

Central and Eastern European (CEE) societies are largely urbanised. The nature of the region's demographics and public health is therefore intimately connected with the physical, environmental and social characteristics of the urban setting.

Therefore, this research theme explores the population and health geography of CEE cities within the context of post-communist transformation by studying a number of topics that are crucial to the understanding of population decline in the region, including:

  • Health and the living environment (including housing);
  • The origins and causes of urban population shrinkage;
  • The roots, patterns and processes of ethnic and socio-economic residential segregation;
  • Housing quality and housing inequalities;
  • Social processes and urban social infrastructure.

The research theme's methodological direction is primarily based on quantitative approaches. The main data sources are the Stakhanov and Yaroslavl Health Interview Surveys (SHIS2009 and YHIS2011), as well as some other survey data. Moreover, historical studies aimed at extrapolating the role of socialist-era legacies in the current social and spatial residential structures of the post-communist city make extensive use of archive sources.

Psychological aspects of health

One topic of special interest for SCOHOST is the investigation of Eastern and Central Eastern European suicide mortality, especially its changes during the transition period (see Mäkinen 2000), but also a more general treatment of the topic of suicide from a social point of view (see Mäkinen & Wasserman 2003; Mäkinen 2009).

The specific themes also include the history of suicide research in Russia (see Vågerö (ed.) 2002), historical changes in Russian suicide mortality (Stickley et al. 2011), Eastern European attitudes towards suicide (Mäkinen 2007, Jukkala & Mäkinen 2011), social geography of suicide (Mäkinen 2006), as well as other questions of general suicide epidemiology (Mäkinen & Stickley 2006; Razvodovsky & Stickley 2009).

Research partners in this area of research include several suicide researchers from both Eastern and Western Europe. In 2010, SCOHOST organised an expert symposium in Tallinn on the topic of Eastern European suicide research, together with the International Institute for Applied Systems Analysis (IIASA) in Vienna and the Estonian-Swedish Suicidology Institute (ERSI) in Tallinn.

Another topic in the field of mental health is depression (Aluoja et al. 2004), on several works are currently in progress.

Surveys and Data

The Changing Life Course Regimes (CLiCR)

The Changing Life Course Regimes (CLiCR) data is a set of variables harmonized by SCOHOST in co-operation with Sunnee Billingsley (Stockholm University) and is constructed with publicly available data sources covering 19 post-socialist contexts: Demographic and Health Surveys (DHS), Family and Fertility Surveys (FFS), Generations and Gender Surveys (GGS) and the Life in Transition Survey (LiTS). Life course histories, particularly related to fertility and partnerships, were constructed for cohorts that came of age in the decades before and after the fall of the Berlin Wall and Soviet Union. In total, CLiCR harmonizes information from 38 data sources (two per country). The total number of men and women included in CLiCR is 152,843. The earliest survey administered was the Polish FFS in 1991, which includes 18+ age range and the latest survey administered was the Albanian DHS in 2008/9, which covered the 15-49 age range. This is the first resource that harmonizes large publically available histories and these data provide new research opportunities on post-socialist countries. The scope of contexts covered in the CLiCR data, including countries as diverse as the new EU member states and Central Asian Republics, make fuller use of the heterogeneity in life course patterns across the socialist and post-socialist region.

Researchers interested in using CLiCR must first gain access to DHS, FFS and GGS data independently.

Moscow Health Survey 2004

The Moscow Health Survey 2004 was designed to examine health among Muscovites in relation to behavioural, economical and social factors. The survey is the result of a collaborative project between SCOHOST and the Institute of Social and Economic Studies of Population (ISESP) at the Russian Academy of Sciences in Moscow.

During the spring of 2004, face-to-face interviews were conducted with 1200 Muscovites using a structured questionnaire. The survey contained approximately 100 questions divided into five areas: demographic and socio-economic positions, living standard, health, life style and social capital.

A sex and age-stratified random sampling technique was used across the 125 municipal districts of Greater Moscow. The response rate was 47 percent and the final sample consisted of 1190 people aged 18 and above, whose demographic and socio-economic characteristics were broadly representative of Greater Moscow.

The Moscow Health Survey has led to a number of publications in peer-reviewed journals on various topics, such as survey methodology, self-rated health, binge drinking, smoking, violent victimisation, attitudes toward intimate partner violence against women, acceptance of suicide and institutional trust, looking at determinants such as gender, education, economic situation and various forms of social capital.

Stakhanov Health Survey 2009

The Stakhanov Health Interview Survey 2009 (SHIS 2009) was completed in June 2009. With a sample of 3,000 respondents, it is one of the largest health and population surveys on individual cities in Central and Eastern Europe. The topics covered range from general physical and mental health indicators, to issues relating to population fertility and mortality, housing, trust in societal institutions, and migration.

Stakhanov is a small city (approx. population 80,000) in the East Ukraine Donbass coal-mining region. With the demise of the Soviet-style command economy, its industrial base has become obsolete, leaving a large share of the city's labour force redundant. The 2008-2009 global economic crisis further accentuated the gravity of the crisis associated with the city's post-Soviet economic restructuring process. As a result, our survey may be said to reflect developments that take place during times of extreme economic turmoil.
